If you do not currently have access to the client portal, print and complete the Client Designee Assignment Form to designate an individual to administer access within your organization. If you need to allow others within your organization access to the portal, print and complete the Client Access Form. These forms should be scanned and emailed to your account manager or faxed to (513) 248-3079.
